Try to use your real name or a short nickname. No lastnames will be added.

Note! Your email can be hidden, but I need it to let you know you were added and if there's been any changes (if we move or similar). Therefor, give me a valid one.

If you have a website and want your site to be viewable in the memberslist, you need to put up a button which is linking back to this site.

If you copy and past the codesnippet to your html-document found in the button-area, and download a button to your harddrive (and then upload everything to your server) then your site will be listed.
However you'll of course be added as a member anyways, if you don't save a button and link it back.
